What you need to bring when you apply:
Proof of Social Security Number of Permanent Residence card (Green Card) for each member of the household
Proof of household’s members income from the prior month.
Current heating bill, or statement from your landlord if they are included in your rent, or statement from utility company if you participate in Pre-Pay.
The account number and name on the account for main heating fuel sources and electric bill.
